Reports to: Learning Commons Supervisor, Critical Path Learning Center
Philadelphia FIGHT Community Health Centers provide culturally-competent comprehensive primary care, and state of the art HIV primary care to low-income members of the community, along with research, consumer education, advocacy, social services, and outreach to people living with HIV and those who are at high risk, including family members, communities with high rates of HIV, formerly incarcerated persons, and young people at risk; and access to the most advanced clinical research in HIV treatment and prevention.
The Critical Path Learning Center at Philadelphia FIGHT is an educational hub and welcoming space devoted to the intersection of health and literacy for the digital age. We nurture life-long learning through meaningful, relevant, non-judgmental education for all people in the community, including people living with HIV, hepatitis C, and other chronic health conditions. Our core programs include a health resources library with open access computers and free wifi; adult basic, GED, and digital literacy instruction; and workforce services.

M-F 9am to 5pm
Tasks and Responsibilities
The Case Manager will provide case management to Learning Center participants on both a drop-in and by appointment basis. Participants may be connected to Learning Center Case Management services through the Learning Commons/Library space of the Learning Center, by the adult education instruction team, or by the workforce development team. The Learning Center Case Manager will work across Learning Center programs to ensure participants have opportunities to set their own goals, develop individualized support plans, and access the resources they need.
Core Responsibilities:
Provide one-on-one support to Learning Center drop in users about accessing vital services, including medical care, mental health care, drug and alcohol, housing, legal, and employment programs.
Maintain 15 drop in case management hours/week.
Work with Learning Center participants to develop individualized learning plan and set goals.
Assist individuals in the Learning Center to use technology to access benefits and get other important services.
Refer and link Learning Center participants to resources, adult education, and job readiness support within the Learning Center.
Refer and link Learning Center participants to health care and other services across FIGHT and other agencies.
Conduct intake interviews and assessment with new Learning Center participants.
Work in partnership with the Adult Education Supervisor and instructors to ensure adult learners have access to wrap around support services.
Work with Learning Commons Supervisor to provide workshops to Learning Center participants on relevant topics, such as getting an ID, securing housing, childcare, etc.
Work with Learning Commons Supervisor to ensure Learning Center participants are able to access amenities including: hygiene kits, clothing closet, and laundry services.
Maintain all documentation through service logs and the entry of notes related to the delivery of case management services to the client in a client record, which is confidential and kept in a locked file cabinet or with the social services database on a secure network.
Ensure all required data are collected and submitted on time.
Attend case conference and trainings conducted by the Lax Center Medical Case Management team.
Provide field supervision for 1-2 MSW interns each year.
Attend team, Education department, and all-staff meetings as required.

Ability to continuously stand, walk, or sit. Ability to bend frequently. Ability to lift up to 10 pounds. Ability to squat occasionally.
Qualifications
Education/Experience:
MSW or equivalent degree required. Minimum of 2 years of experience providing case management or similar service to low-income and marginalized populations. Experience in adult education or workforce development highly desirable. Excellent reading skills and deductive reasoning skills; excellent communications, interpersonal skills and collaborative skills; flexible, self-motivated, organized, able to work well under pressure; computer literacy required; Must be able to provide LGBTQ and trans-competent and sensitive services.
EOE Additional Learning Center Support Responsibilities:
Staffing the Learning Center space as needed. Work with the Learning Commons Supervisor to maintain the public Learning Center space, including tidying up, instructing patrons to adhere to the rules, and creating a warm environment for all. Work with other Learning Center staff to run monthly orientation sessions for Learning Center clients. Maintain all statistical gathering, data entry, and reporting requirements as dictated by organizational need. Occasional office duties such as creation and posting of fliers, copying of materials, and picking up supplies. Assist with AIDS Education Month –attending event planning meetings, calls, emails, assisting with event coordination, and attending all events during June. Assist and support other Education Department programs as necessary.
