Part-time Reentry Specialist with Justice Works Youth Care

Position Summary: The Re-Entry Specialist will maintain a caseload of referrals from our contracting agencies to provide services to clients related to safety and objectives in accordance with referral information, assessments and ongoing needs. Position requires non-traditional business hours (nights, weekends and some holidays). Duties & Responsibilities:

  • Engage families in service and treatment planning.

  • Assess family and client needs.

  • Conduct a minimum of one community visit per week with all clients who have been identified by the contracting agency. These sessions may take place in the home, community or office. Two visits per month must occur in the home.

  • Plan and implement appropriate interventions using evidence based and best practice approaches.

  • Enter and maintain all aspects of client information including required service notes in database system within 24 hours of service.

  • Evaluate the effectiveness of service provision.

  • Follow up as needed to determine appropriate courses of action.

  • Meet with designated clients for the prescribed number of hours/days/contacts per week.

  • Maintain open communication with referring authorities.

  • Attend court hearings and provide testimony as required.

  • Perform other duties as needed.

Requirements:

  • Bachelor’s Degree from an accredited institution in social work, criminal justice, psychology, or related field and a minimum of one (1) year direct care experience.

  • Valid driver’s license, auto insurance and access to a vehicle.

  • Pass current, applicable clearances.

  • Pass pre-employment, post-offer drug screening and TB
