We are currently seeking compassionate and dedicated professionals to join our Service Coordination team. The Service Coordinator will work on a team to deliver quality support and coordination of services to adults who are diagnosed with serious and persistent disabilities. These services and supports are based on the principles of recovery and self-determination.
Responsibilities and Duties Primary Responsibility
Provides quality customer service, including interacting with customer, answering customer inquiries, and effectively handling customer complaints
Responsible for assessment and planning eligible services
Coordinate and monitor needs of service and support through the development and implementation of the Individual Support Plan (ISP)
Provide phone and in-home assessments to monitor and support consumers
Assist individuals receiving services; identifying and communicating requests and needs for services and support, including, but not limited to: housing services; medical services; social services; & educational services
Bachelor's Degree Required
