The Forensic Intensive Recovery (FIR) Program is a prison deferral initiative that offers eligible criminal offenders substance abuse treatment in lieu of incarceration. PHMC is responsible for operating the Clinical Evaluation Unit, which evaluates eligible criminal offenders for chemical dependency; refers them to community based providers for residential, intensive outpatient and regular outpatient treatment services; and provides case management services for FIR program participants. Coordinate supportive services for a caseload of 50 or more individuals in Philadelphia Drug Treatment Court. Work location 1401 Arch St. in Center City Philadelphia.
Responsibilities:
Providing case management services to chemically dependent participants of Philadelphia Treatment Court.
Documentation of service provision within 48hrs of service completion to participants assigned.
Case Managers will ensure the delivery of comprehensive services to assigned participants in the areas of housing, education, vocation, mental health, medical, child welfare, domestic violence, legal and recovery oriented community services.
Participation in bi-weekly supervision meetings.
Case Managers are responsible for meeting with participants monthly or more if clinically necessary.
Monthly site visits to treatment program are required in an effort to maximize coordination of care.
Serving as liaisons between the treatment facility and legal team.
Case Managers will prepare accurate and timely progress summaries and present at the bar of the court.
Submission of documentation for reauthorization of services for individuals on caseload
Completion of discharge summaries within timeframes specified by the program.
Maintain participant records in accordance with agency fee for service standards, managed care, and licensing requirements
Prioritizing assisting clients to enroll in treatment, physical health services, DPA, vocational services, and educational services.
Identify, coordinate, and link community resources to client to promote accessibility to recovery oriented community services
Submit statistical information as requested by supervisor
Maintain PAJCIS compliance and accountability with current participant information (DAP formatted notes)
Case Managers will play a key role in developing intra and inter organizational linkages, fostering collaborative relationships with various programmatic entities.
Case Managers will conduct urine drug screens for Treatment Court participants as required.
Case Managers will regularly meet with participants in treatment facilities, recovery houses and follow up on court ordered sanctions or requests and phase calculations.
Case Managers will perform all other duties as assigned.

Preferences for this opportunity include experience in: case management services with participants with forensic backgrounds who have abused illicit psychoactive substances; and experience in courtroom presentations.
Experience:
Two (2) years of direct contact with participants in a substance abuse treatment setting preferred.
Education Requirement:
Minimum Bachelor’s Degree in a related field.
Case Managers must complete required Case Management trainings provided by BHTEN within one year of hire.
Pennsylvania Client Placement Criteria (PCPC) training to be completed within one year of being hired.
