Responsibilities:
Provide case management to STRIVE students to ensure successful program participation and completion of 85%of case load
Participate in two week pre assessment phase and program orientation
Identify needs of Welcome Home Kits
Distribute welcome home kits to participants
Conduct cross system intake assessment to assess student status and needs in the areas of education, including any gaps or learning differences, employability, housing status, parenting status, career and other interests, mental health status, and overall health status
Develop strengths-based Individual Development Plan outlining all planned services for the students including the Individual Career Plan and occupational skills training
Coordinate with probation, reentry services, and other stakeholders and systems to collect and communicate information regarding student progress and to coordinate needed supports
Establish and schedule meetings with 2-3 legal service organizations
Establish at 5-7 partnerships with community based organizations
Research and attend other local city/state funded re-entry programs
Create standing days/time for presentations based on organizations availability
Create calendar of events
Work with staff to obtain and report attendance and performance records
Liaise with Probation Officers, staff ,etc., regarding student’s performance, including tracking occupational skill training/credential attainment
Conduct home and site visits as necessary
Facilitate and supervise academic supports for students including educational help and academic enrichment around literacy and numeracy
Facilitate Moral Recognition Therapy workshops
Facilitate instructional project activities for students
Connect students to GED or higher education programs as needed
Coordinate mentoring relationships for students based on career pathways/interest
Coordinate student support from the Job Developer to ensure appropriate employment placement and retention based on identified career pathways
Recommend internships based on occupational skills training identified during development of individual career plans
Meet weekly one-on-one with students to monitor program progress, housing, childcare, transportation, mandatory reporting requirements, and any barriers that may prevent successful completion of program
Maintain current information on resources relevant to student needs
Provide students with transportation and other incentives for program participation
Provide post placement case management services on a biweekly basis for the first three months following employment placement, then monthly for the next six months to ensure sustained employment
Enter all case notes, attendance and any additional information into required data system/s in a timely manner
Maintain accurate student records
Obtain all necessary eligibility program requirements prior to core STRIVE program
Monitor student daily program attendance
Assist in completing reports on student participation
Complete required benchmark paperwork for successful completion of program
Assist in coordinating student orientation, motivational sessions and special programs for students
Participate in and report on each student based on assigned case load during weekly Direct Service Team meetings (DST)
Update referring agencies on progress/challenges of each student on assigned case load
Support recruitment efforts for funding projects
Complete additional program duties and/or assignments as requested by Supervisor
Skills:
Case management experience preferred.
Experience facilitating group activities for youth.
Strong knowledge of legal and juvenile justice systems.
Strong computer skills, especially in Microsoft Word and Excel
Experience using databases.
Experience:
Minimum of two (2) years experience working with economically and educationally disadvantaged youth, preferably youth involved in the juvenile justice system. (indicate education level required for the job)
Education Requirement:
Bachelor’s degree in Social Work or relevant field.
