Fruits of the Family Table Looks to Hire Case Manager

Fruits of the Family Table, FOTFT is a 501(c)3 Nonprofit who aims to restore the family unit, by addressing the areas of substance use disorders, parenting, senior care and (re)integration, with the use of therapeutic services, education, community events, mentorship and advocacy, we at FOTFT (Fruit of the Family Table). We envision a Delaware and Philadelphia county in which all communities are free from the plagues that bound them in the areas of substance use, mass incarceration, parenting and abuse/neglect of the aging.

As Fruits of the Family Table continues to expand, we are seeking an experienced Re-entry Case Manager to work a 6-9-month program with men and women in an employment training program. This is a fee for service part-time position.

FOTFT Case Management Philosophies

Case management is based on the premise that when one reaches their highest level of functioning, everyone wins: the individuals being served and their families, thus trickling down into the community at large. The essential case management functions include; resource allocation, advocacy, engagement and evidence-based practice implementation.

Experience & Education Required

The candidate whom we are seeking needs to have at least 3 years of experience in the behavioral health field, preferably re-entry. They must have acquired at least an associate degree from an accredited university in a related field, such as behavioral health, sociology or criminal justice. This candidate must also be familiar with local resources and can allocate and network when needed. We are open to hiring someone with lived experience.

Some Duties Include:

  • Maintains a case load of 15-20 individuals

  • Conducts bi-weekly and monthly visits with program participants

  • Compose individual service plans for each participant

  • Compose and upkeep of electronic charts

  • Attends all mandatory FOTFT trainings and staff meetings, including supervision

  • An ability to self-govern, in addition to working in an interdisciplinary team

  • Facilitate and/or co-facilitate workshops on life skills with participants

  • Contribute to staff meetings and case consultations

  • Conduct site visits with participants who require other measures of engagement

  • Can identify and align with FOTFT values, mission and vision
