As Philadelphia’s hub for reentry referrals, resources, and training, the goals of the Office of Reentry Partnerships are to:
1) Set a clear vision and direction for a measurable citywide approach to improving reentry outcomes
2) Drive and sustain an unprecedented level of coordination between the local government, other government partners, service providers, education and training programs, employers, and community members
3) ensure that City reentry initiatives are research- and data-driven
Salary Range: $50,000 - $55,000
Job Description
Provide comprehensive case management services for the adult reentry participants as they navigate the reentry process. This position will provide case management and transitional management to persons currently incarcerated as well as persons who have been released from incarceration and need a continuum of care. Work with participants using and Integrated Case Management process that will orient and assess participant readiness for programming, identifying strengths and challenges to achieving goals and collaborating with participants in formulating a strong reentry plan. Work closely with other program staff both in the jails and prisons as well as other community partners to monitor progress, and coordinate referral support. This position will also work with external partners (such as literacy, behavioral health and housing providers, benefits, etc.) to ensure that services are coordinated, and participants are able to fully engage in a holistic reentry process.
Qualifications:
Bachelor’s degree or equivalent work experience
A minimum of years of directly relevant experience
A minimum of 2 years working with prisons impacted by mass incarceration
Spanish speaking
Preferred:
People who have been directly affected by incarceration are encouraged to apply
Prior experience working within a criminal justice system
