The Social Security Card Center is now re-opened. As of April 18, 2022, this is the latest update on obtaining replacement Social Security card.

1. Complete Social Security App online at: (https://www.ssa.gov/forms/ss-5.pdf). (Advise having completed app on-hand when arriving at SS office.)

2. Bring completed app and valid ID to Social Security Card Center Office @ 2 Penn Center, 1500 JFK Blvd; Open 9 a.m. - 4 p.m. M-F.

3. Client will receive a confirmation receipt from SSA (sample attached) and should have card w/in two weeks.
